I suppose you mean something to give the client, for them to fill out. I do not use that. I simply keep name, addess, phone number, date of birth, and I check with spirit what their first and second spiritual gifts are. (this is the information of what two elements they chose before birth). From the date of birth, I compute the numbers for the numerology that follows the book, "Live the Life you were born to Live". I share these last two pieces of information with them because I believe it is important that a client knows everything that I know about them. You will note that I do not ask about physical problems as this will come through reiki itself.
